DECLARATION OF TRUTHFULNESS

I hereby declare that the information given above and in all accompanying documents is true, complete, and accurate to the best of my knowledge and belief, and that nothing material has been concealed or omitted. I understand that any false, misleading, or incomplete information may result in the rejection of my application or termination of my volunteer service. I acknowledge that Urban Seed relies on this information in making decisions regarding my volunteer placement and that I have a duty to update Urban Seed if any information changes.

ASSUMPTION OF RISK AND RELEASE OF LIABILITY

I understand and acknowledge that by volunteering with Urban Seed, I am voluntarily participating in activities that may involve certain inherent risks, including but not limited to:



Physical injury from volunteer activities, equipment use, or environmental conditions

Property damage or loss to personal belongings

Exposure to weather conditions during outdoor activities or events

Interaction with community members who may be experiencing various challenges

Transportation risks when traveling to/from volunteer sites

Exposure to communicable diseases or other health risks

Handling of materials, tools, or equipment that may cause injury

Working in various environments including but not limited to community gardens, events, and public spaces



RELEASE AND HOLD HARMLESS

In consideration of being accepted as a volunteer with Urban Seed, I hereby:



VOLUNTARILY ASSUME ALL RISKS associated with my volunteer service, whether known or unknown, foreseen or unforeseen.

RELEASE AND DISCHARGE Urban Seed, its board of directors, officers, employees, volunteers, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising from or related to any injury, loss, damage, or harm that may occur during my volunteer service.

HOLD HARMLESS Urban Seed from any claims by third parties arising from my volunteer activities or participation in Urban Seed programs.

WAIVE ANY RIGHTS to seek compensation or damages from Urban Seed for any injury, loss, or damage I may sustain while volunteering.

AGREE TO INDEMNIFY Urban Seed against any claims, damages, or expenses (including attorney's fees) arising from my volunteer activities or any breach of this agreement.



INSURANCE AND RESPONSIBILITY


I understand that Urban Seed does not provide health, disability, or liability insurance coverage for volunteers

I am responsible for maintaining adequate personal insurance coverage

I participate as a volunteer at my own risk and expense

I am responsible for my own medical expenses in case of injury



VOLUNTEER ACKNOWLEDGMENTS


I understand that I am volunteering my services without expectation of compensation

I agree to follow all Urban Seed policies, procedures, and safety guidelines

I will immediately report any unsafe conditions or incidents to Urban Seed board members

I understand that my volunteer service may be terminated at any time by either party

I grant permission for Urban Seed to use photographs or videos of me taken during volunteer activities for promotional purposes

I agree to maintain confidentiality regarding sensitive information I may encounter while volunteering



EMERGENCY MEDICAL TREATMENT

I authorize Urban Seed board members to obtain emergency medical treatment for me if I am injured and unable to communicate. I understand that Urban Seed will make reasonable efforts to contact my emergency contacts, but that treatment will not be delayed for lack of contact.




I HAVE READ AND UNDERSTAND THIS RELEASE OF LIABILITY AND HOLD HARMLESS AGREEMENT. I UNDERSTAND THAT BY SUBMITTING THIS FORM, I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE URBAN SEED FOR DAMAGES. I AGREE TO THESE TERMS VOLUNTARILY AND WITH FULL KNOWLEDGE OF THEIR LEGAL SIGNIFICANCE.
